Active Case Management

Cognate Health has developed an Active Case Management service. Our Case Managers are a mix of occupational health nurses and physiotherapists with a special interest in rehabilitation. Our active case management process is an early intervention process that allows occupational health services to support sick/absent employees and provide advice to line management and HR on rehabilitation in the workplace thereby ensuring that all available resources are used to enable a return to optimum health and work.

The process is as follows:

 

THE LINE MANAGER makes a referral for case management and confirms that the employee has been made aware of the referral.

 

THE CASE MANAGER:

a. Makes phone contact with the employee.
b. Updates the referring manager after the telephone consultation.
c. Escalates difficult cases to the occupational health physician.
d. Has regular meetings with HR to discuss live cases.
e. Arranges a face to face consultation with the OHP when appropriate.

 

REASONS FOR REFERRAL INCLUDE:

a. Frequent short-term sickness absence
b. Long-term sickness absence
c. Medical review of disclosed health issue
d. Health-related performance issue
e. Possible work-related health problem
f. Accident / Injury at work
g. Other

 

THE CASE MANAGER CAN REDUCE THE WORKLOAD OF HR AND LINE MANAGEMENT BY PERFORMING THE FOLLOWING TASKS:

a. Keeping in regular contact with the employee e.g. weekly.
b. Arranging appointments for physiotherapy, specialist opinion, employee assistance
program.
c. Following up with the employee to ensure attendance at appointments.
d. Making contact with treating clinicians such as the GP.
e. Liaising with line management with regards to exploring the possibility of
“Reasonable Accommodations” in the workplace.

 

THE BENEFITS TO THE EMPLOYEE ARE AS FOLLOWS:


a. Single point of contact i.e. with the case manager
b. They receive telephone advice on ways to augment their current treatment.
c. We use evidence-based treatment protocols for a wide range of conditions.
d. We have access to a range of allied health services such as counselling,
physiotherapy, dietetics etc through our health and wellbeing partners.
e. We facilitate maximum treatment to enable the earliest possible recovery and
return to work.
